ILARIS^® Patient Support Program for Patients With CAPS (Cryopyrin-Associated Periodic Syndromes)

Familial Cold Autoinflammatory Syndrome (FCAS) and Muckle-Wells Syndrome (MWS) are 2 conditions in a group of rare diseases called CAPS.¹

• Although there may be only about 300 patients with CAPS in the United States, experts believe that not all patients with CAPS have been diagnosed yet because of poor disease recognition^1,2

For patients with CAPS, the ILARIS® Patient Support Program has been designed by Novartis Pharmaceuticals to offer support through the challenges they may face. Read on to learn about our free support program and services.

Assistance With the Insurance Process
The ILARIS® Patient Support Program provides patients with information and tools to assist in managing their treatment with ILARIS® and helps to identify resources that can assist with payment for their medication, including:

• Contacting their insurance companies
• Verifying insurance benefits and investigation of coverage
• Assisting with prior authorizations and appeals, if needed
• Providing educational materials
• Referring patients to a co-pay assistance program that may help reduce eligible patients' out-of-pocket costs, such as the National Organization for Rare Disorders, also known as the NORD Premium and Co-pay Assistance Fund

If Your Patient Does Not Have Insurance
If your patient has no insurance or their insurance does not cover treatment with ILARIS® (canakinumab), additional patient support may be available for those who qualify. You may refer them to other resources, such as:

• The Novartis Patient Assistance Now Program, which is an easy-to-use resource that offers Novartis drugs free of charge to those who qualify, as well as additional information about how to take care of their health
• Partnership for Prescription Assistance (PPA). The ILARIS® Patient Support Program can also refer potentially eligible patients to the Partnership for Prescription Assistance. The mission of PPA is to increase awareness of patient assistance programs and boost enrollment of those who are eligible. The PPA offers a single point of access to more than 475 public and private programs, including nearly 200 offered by pharmaceutical companies

ILARIS® (canakinumab) is only available through a specialty pharmacy provider network. Ordering ILARIS® for your patients is easy.

• Step 1. Simply download the ILARIS® Service Request Form (SRF)
• Step 2. Complete the form and review it with your patient
• Step 3. Fax the signed form back to 1-866-972-8316 to enroll patients in the Patient Support Program
