From Diagnosis to ILARIS®
If you experience symptoms associated with CAPS, talk to your doctor about your CAPS Symptoms.
If a physician diagnoses you with CAPS and decides that ILARIS® (canakinumab) is right for you, he or she will start the process by prescribing ILARIS® .
Getting Started With ILARIS®
ILARIS® is not available at regular pharmacies. Your doctor will start the process by following these steps:
- Step 1. Your doctor will download and complete a form for ILARIS® called the Service Request Form (SRF).
- Step 2. He or she will review the completed SRF with you, sign it, and fax the form to the ILARIS® Patient Support Program.
The ILARIS® Patient Support Program will then continue the process.
- Step 3. If you have insurance, the ILARIS® Patient Support Program will contact your insurance company to find out if you are covered.
- Step 4. The ILARIS® Patient Support Program will then contact you and your doctor to discuss your benefits and request any authorizations or other information that may be needed. If you are in need of financial support, the ILARIS® Patient Support Program may refer you to other resources that you may be eligible for.
- Step 5. If your ILARIS® prescription is covered, then the ILARIS® Patient Support Program will work with your doctor and a specialty pharmacy to set up shipment of your prescription.
